Sudden Onset of Knee Pain1

A seventy-five-year-old woman, who was otherwise in good health, reported the sudden onset of severe knee pain. On examination she was afebrile. There was a mild knee effusion but no erythema. Her range of motion was limited by pain; there was no malalignment or crepitus. The knee was radiographically normal. After treatment with anti-inflammatory agents, she reported some symptomatic improvement, which proved to be temporary. Seven months later, she returned with persistent knee pain.
