An operative procedure for correction of overlapping fifth toe consists in running the distal stump of the extensor longus tendon, with its insertion intact, through an oblique spiral channel across the plantar surface of the toe, and implanting it into the conjoined tendon of the abductor and short flexor of the fifth toe, after dorsal and medial capsulotomy of the fifth metarsophalangeal joint. The operative procedure seems to be mechanically sound, because it not only corrects the main three components of the deformity, but also utilizes the muscle power of the abductor and short flexor for active maintenance of the correction.
