The abductor digiti quinti isolated on its neurovascular pedicle can be transposed to serve as an effective substitute for the abductor pollicis brevis to restore thumb opposition. The method, although technically difficult, is applicable whenever the deficit does not include the hypothenar muscles; and it may offer the only dynamic means of bringing the thumb into functional concord with the fingers in situations in which other motor transfers are unavailable or needed for other purposes.
