The fact has been firmly established that intertrochanteric osteotomy will relieve the pain of degenerative arthritis of the hip. The bone fragments may be angulated in various directions and rotated to improve the congruity of the joint surfaces, reduce muscle tensin, and correct gross angular deformity. The fragments may be displaced to correct bow-leg or to establish a neo-arthrosis under the acetabulum and thus remove part of the force of weight-bearing from the femoral head. If the osteotomy is to succeed in reversing the degenerative process and restoring function, all of the suitable components of the operation should be carried out. It remains for the orthopaedic surgeons of the future to establish proof of the modus operandi and then to simplify the technique of this excellent surgical procedure for the relief of painful osteo-arthrosis of the hip.
