The two most common methods for fixation of a humeral diaphyseal fracture are plate osteosynthesis and intramedullary nail fixation. Implants available for intramedullary fixation consist of flexible nails and rigid nails with or without interlocking. Flexible nails have ease of insertion as an advantage, but they do not provide rigid fixation with respect to angulation, distraction, or rotation. Rigid interlocked nails address these problems; however, because of their rigidity, they are more difficult to insert and they may violate the rotator cuff during antegrade insertion.
